W postępowaniu mogą brać udział Wykonawcy, którzy posiadają doświadczenie niezbędne do wykonania zamówienia za spełnienie przedmiotowego warunku, Zamawiający uzna wykazanie się przez Wykonawcę realizacją w okresie ostatnich 3 lat przed upływem terminu składania ofert, a jeżeli okres prowadzenia działalności przez Wykonawcę jest krótszy, to w tym okresie, co najmniej 2 dostaw/zamówień, których przedmiotem zamówienia była dostawa i uruchomienie urządzenia typu: system sterowania linią do nanoszenia nadruków na szkle płaskim.

Only Contractors who have the experience necessary to perform the contract may participate in the procedure the Ordering Party shall recognize that the condition in question has been satisfied if the Contractor can prove that in the period of 3 years prior to the expiry of the quotation submittal date, and if the Contractor’s business activity period is shorter then within this period – of at least 2 deliveries/contracts whose subject matter was the delivery and commissioning of such a device as control system for the flat glass printing line.
Potencjał techniczny
W postępowaniu mogą brać udział Wykonawcy, którzy będą dysponować na czas realizacji zamówienia zasobami technicznymi koniecznymi do prawidłowej realizacji wykonania zamówienia.

Only Contractors may participate in the procedure who will have the technical capacity required for the correct performance of the contract at their disposal during the contract performance period.
Lista wymaganych dokumentów/oświadczeń
1. Oferta składana przez Wykonawcę powinna być sporządzona na formularzu stanowiącym Załącznik nr 2 do Zapytania ofertowego „Wzór formularza oferty”.
2. Zamawiający zaleca wykorzystanie formularzy przekazanych przez Zamawiającego. Dopuszcza się złożenie ofert na formularzach opracowanych przez Wykonawcę pod warunkiem, że będą one zawierały wszystkie wymagane przez Zamawiającego informacje i oświadczenia, zaś w przypadku ich ręcznego wypełnienia – pod warunkiem, że będą wypełnione w sposób czytelny.
3. Oferty należy sporządzić w języku polskim lub języku angielskim. Dokumenty sporządzone w innym języku niż polski lub angielski powinny być składane wraz z tłumaczeniem na język polski lub język angielski.
4. Oferta i załączniki do oferty muszą być podpisane przez upoważnionego(ych) przedstawiciela(i) zgodnie z formą reprezentacji Wykonawcy określoną w dokumencie rejestrowym (ewidencyjnym) Wykonawcy lub przez pełnomocnika Wykonawcy.
5. Zamawiający zaleca, aby każda zapisana strona oferty (wraz z załącznikami do oferty) była ponumerowana kolejnymi numerami.
6. Do wypełnionego Formularza oferty należy dołączyć:
6.1 opis techniczny Przedmiotu zamówienia Wykonawcy wraz z rysunkiem technicznym zawierającym wymiary gabarytowe elementów wykonawczych systemu Przedmiotu zamówienia wskazane w pkt. 3.4. Zapytania ofertowego. Oba dokumenty przygotowane na wzorze własnym wykonawcy,
6.2 wypełnioną Specyfikacje techniczną przedmiotu zamówienia oferowanego przez Wykonawcę zgodnie z wzorem stanowiącym Załączniku nr 5 do niniejszego Zapytania Ofertowego „Specyfikacja techniczna urządzenia oferowanego przez Wykonawcę;
6.3 Oświadczenie o braku podstaw do wykluczenia z postępowania zgodnie z wzorem zawartym w części „Oświadczenie Wykonawcy dot. braku powiązań z Zamawiającym” Załącznika nr 2 do Zapytania ofertowego „Wzór formularza oferty”;
6.4 Oświadczenie o wypełnieniu obowiązków informacyjnych przewidzianych w art. 13 lub art. 14 RODO, którego treść uwzględniona została w Załączniku nr 2 do Zapytania ofertowego „Wzór formularza oferty”;
6.5 Wykaz wykonanych zamówień zgodnie z wzorem stanowiącym Załącznik nr 3 do Zapytania ofertowego;
6.6 W przypadku, gdy ofertę w imieniu Wykonawcy podpisuje pełnomocnik (osoba nieumocowana do tych czynności w dokumentach rejestracyjnych) Wykonawcy należy do oferty dołączyć skan stosownego pełnomocnictwa.

1. The quotation submitted by the Contractor should be prepared on the form attached as Annex 2 to the “Quotation Template”.
2. The Employer recommends using forms provided by the Employer. It is allowed to submit quotations on the forms prepared by the Contractor provided that they contain all the information and declarations required by the Employer, and where they are filled out by hand – provided that they are filled out in a legible manner.
3. Quotations should be prepared in Polish or English. Documents made in language other than Polish or English shall be submitted along with translation into Polish or English.
4. The quotation and attachments thereto must be signed by the authorised representative(s) in accordance with the Contractor’s authority for representation set out in the Contractor’s registration documents (registry documents) or by the Contractor's attorney-in-fact.
5. The Employer recommends that each page of the quotation (including attachments thereto) should be numbered with consecutive numbers.
6. The completed Quotation Form should be accompanied by:
6.1 technical description of the subject of the contract as well as a technical drawing showing the overall dimensions of the actuating components of the system to delivered as the subject of the contract indicated in point 3.4 of the Request for Quotation. Both documents prepared on the Contractor's own template,
6.2 completed Technical Specification of the subject of the contract offered by the Contractor in accordance with the template attached as Annex 5 to this Request for Quotation “Technical Specification of the Equipment offered by the Contractor”;
6.3 Declaration on satisfying information obligations provided for in Article 13 or Article 14 of the GDPR, the content of which has been taken into account in Annex 2 to the Request for Quotation “Quotation Form Template”;
6.4 Declaration on the lack of grounds for exclusion from the procedure in accordance with the template of the “Contractor’s Declaration on the lack of ties with the Ordering Party” provided for in Annex 2 to the Request for Quotation – “Quotation Form Template”;
6.5 List of Completed Contracts in accordance with the template attached as Annex 3 to the Request for Quotation;
6.6 In the event that the quotation is signed on behalf of the Contractor by an attorney-in-fact (a person not authorized to perform these activities in the Contractor’s registration documents), a scan of the relevant power of attorney should be attached to the quotation.
Dodatkowe warunki udziału
1. Z udziału w postępowaniu ofertowym wykluczeni są Wykonawcy, w stosunku do których zachodzą powiązania kapitałowe lub osobowe. Przez powiązania kapitałowe lub osobowe rozumie się wzajemne powiązania pomiędzy Zamawiającym lub osobami upoważnionymi do zaciągania zobowiązań w imieniu Zamawiającego lub osobami wykonującymi w imieniu Zamawiającego czynności związane z przeprowadzeniem procedury wyboru Wykonawcy a Wykonawcą , polegające w szczególności na:
a) uczestniczeniu w spółce jako wspólnik spółki cywilnej lub spółki osobowej,
b) posiadaniu co najmniej 10% udziałów lub akcji,
c) pełnieniu funkcji członka organu nadzorczego lub zarządzającego, prokurenta, pełnomocnika,
d) pozostawaniu w związku małżeńskim, w stosunku pokrewieństwa lub powinowactwa w linii prostej, pokrewieństwa drugiego stopnia lub powinowactwa drugiego stopnia w linii bocznej lub w stosunku przysposobienia, opieki lub kurateli.
2. W celu wykazania braku istnienia podstaw wykluczenia z postępowania o udzielenie przedmiotowego zamówienia, o którym mowa w pkt 1 powyżej, Wykonawca powinien wraz z ofertą złożyć oświadczenie własne „Oświadczenie Wykonawcy dot. braku powiązań z Zamawiającym” zawarte w części Załącznika nr 2 do Zapytania ofertowego „Wzór formularza oferty”.
3. Ofertę Wykonawcy wykluczonego uważa się za odrzuconą.

1. Contractors with capital or personal ties shall be excluded from participation in the contract award procedure. Capital or personal ties shall be understood as mutual relations between the Employer or persons authorized to incur liabilities on behalf of the Employer or persons acting on behalf of the Employer in the performance of activities related to the Contractor selection procedure and the Contractor, including in particular:
a) participating in the company as a partner in a partnership,
b) holding at least 10% of shares or stock,
c) acting as a member of the supervisory or management body, proxy, or attorney-in-fact,
d) being in a marriage, kinship or affinity of the first degree, kinship or affinity of the second degree or affinity of the second degree, or being related by affinity in the collateral line, or being the relationship of adoption, custody or guardianship.
2. In order to prove that there are no grounds for exclusion from the contract award procedure referred to in point 1 above, along with the quotation, the Contractor should submit “Contractor’s Declaration on the lack of ties with the Employer” contained in Annex 2 to the Request for Quotation “Quotation Form Template”.
3. A quotation submitted by an excluded Contractor shall be considered rejected.
